Health information can be accessed under the NSW Health Privacy Manual and the Health Records and Information Privacy Act 2002 (HRIPA). The Health Information Services Department at STARTTS is responsible for providing you with your health records according to the Act.

In order to obtain a full copy of your health record, please complete the following steps:

  • After completing the Application for Access to Health Care Records form, you need to sign and date (within 3 months when the submission of the request ) the form
  • Provide your ID (passport, Australian Driver’s Licence, Immicard or Australian citizenship certificate)

If you are an individual applying for the client’s information on his or her behalf, you need to provide:

  • Your own and the client’s ID
  • Consent from the client dated within 3 months of the request
    • If you are the client’s legal guardian, a copy of the Guardianship Order
    • For a child under age 16, the parent or legal guardian must consent

If you are requested for records of a deceased client, you will need to provide a copy of the will identifying you as the executor or signed authorisation from the executor of the will.

Application forms and fees can be delivered in person to the Health Information Services Department at STARTTS Carramar, or they can be posted to the Department.  Please refer to contact details above for postal address.

The photocopy of the original signed and dated application form is accepted if it is sent via email. Otherwise, the original signed and dated application form is requested and must be either delivered in person or posted before your request is being processed.

A written request needs to be submitted with the following details:

  • full name of the client
  • date of birth
  • present address and, if different, address at the time of the health treatment in question
  • details of the records/information in question
  • range of dates for health treatment in question
  • name of person and organisation being authorised

Please note that all fees are waived for asylum seeker clients and the below information can be disregarded.

A minimum of $33.00 (application fee) is required for clients who request for a full copy of health records.

The fee covers the production of the first 80 pages of the information. There will be an additional photocopying fee ($0.40) per page applied if a request exceeds 80 pages. A cheque or money order (made payable to STARTTS) for the prescribed fee must accompany the application before the information is disclosed to you.

Discount of an application fee

Discount of 50% reduction in application fees only will be applied if:

  • The applicant is the holder of a current Pensioner or Concession card issued by the Commonwealth

Waive of application fee

Clients that are of asylum seeker status are completely waived of any fees associated with release of information requests.

Health Information Services aim to process your request within 21 working days of receipt of valid application form, documentation and fee.

If your request is urgent, you may indicate this in your request. The Health Information Services Department will aim to prioritise your request but please understand that no guarantees can be made.

You may come to STARTTS to collect the photocopy of your record in person or the photocopy can be posted to an address nominated by you. Please note STARTTS does not take any responsibilities for lost mail.

If you request STARTTS to provide you with an attendance or a support letter, you must complete the following:

No fee will be incurred to obtain an attendance or a support letter.

If you wish to view your health records, there will be no charge. However, you need to complete an application form and provide the proof of your ID. You will also need to contact the Health Information Services Department to schedule an appointment to view your record.

During your appointment, a Health Information Services representative will sit with you and respond to your questions regarding your record. However, the representative will not explain any psychological questions.
